Pragmatic Free Trial Meta
Pragmatic Free Trail Meta is an open data platform that allows research into pragmatic trials. It collects and shares cleaned trial data and ratings using PRECIS-2, permitting multiple and varied meta-epidemiological research studies to compare treatment effects estimates across trials that have different levels of pragmatism, as well as other design features.
Background
Pragmatic trials are becoming more widely acknowledged as providing evidence from the real world for clinical decision making. The term "pragmatic" however, is not used in a consistent manner and its definition and evaluation need further clarification. Pragmatic trials must be designed to inform policy and clinical practice decisions, rather than to prove the validity of a clinical or physiological hypothesis. A pragmatic study should aim to be as similar to the real-world clinical environment as possible, such as its recruitment of participants, setting up and design of the intervention,

Methods
In a pragmatic research study the aim is to inform clinical or policy decisions by showing how an intervention can be integrated into routine treatment in real-world contexts. This is distinct from explanation trials that test hypotheses regarding the cause-effect relationship in idealised settings. In this way, pragmatic trials may have lower internal validity than explanation studies and be more susceptible to biases in their design analysis, conduct, and design. Despite their limitations, pragmatic studies can provide valuable information to make decisions in the context of healthcare.
The PRECIS-2 tool evaluates an RCT on 9 domains, with scores ranging between 1 and 5 (very pragmatic). A common aspect of pragmatic studies is that researchers attempt to make their findings more relevant by studying subgroups of the trial sample. This can result in imbalanced analyses and less statistical power. This increases the chance of omitting or ignoring differences in the primary outcomes. This was a problem in the meta-analysis of pragmatic trials because secondary outcomes were not adjusted for covariates that differed at the time of baseline.

Results
While the definition of pragmatism does not require that all clinical trials are 100% pragmatist, there are benefits of including pragmatic elements in trials. These include:
Increased sensitivity to real-world issues which reduces the size of studies and their costs, and enabling the trial results to be faster implemented into clinical practice (by including patients who are routinely treated). Schwartz and Lellouch1 created a framework for distinguishing between explanation-based trials that support a physiological or clinical hypothesis and pragmatic trials that help in the selection of appropriate treatments in the real-world clinical setting. The framework consisted of nine domains scored on a 1-5 scale which indicated that 1 was more lucid while 5 was more practical. The domains covered recruitment and setting up, the delivery of intervention, flexible compliance and primary analysis.
The original PRECIS tool3 was an adapted version of the PRECIS tool3 that was based on the same scale and domains. Koppenaal et. al10 devised an adaptation of the assessment, dubbed the Pragmascope, that was easier to use for systematic reviews. They found that pragmatic reviews scored higher on average across all domains, however they scored lower in the primary analysis domain.
This distinction in the primary analysis domain can be explained by the way that most pragmatic trials analyze data. Some explanatory trials, however, do not. The overall score was lower for pragmatic systematic reviews when the domains of organisation, flexible delivery, and follow-up were combined.
It is important to remember that a pragmatic study should not necessarily mean a low-quality study. In fact, there is an increasing number of clinical trials that employ the term "pragmatic" either in their abstracts or titles (as defined by MEDLINE but which is neither precise nor sensitive). These terms may indicate a greater appreciation of pragmatism in abstracts and titles, but it's unclear whether this is reflected in content.
Conclusions
As appreciation for the value of real-world evidence grows popular the pragmatic trial has gained popularity in research. They are randomized clinical trials that evaluate real-world alternatives to care rather than experimental treatments under development. They have patient populations that more closely mirror those treated in routine care, they use comparators that are used in routine practice (e.g., existing drugs) and depend on the self-reporting of participants about outcomes. This approach has the potential to overcome the limitations of observational studies, such as the limitations of relying on volunteers and the lack of availability and the variability of coding in national registry systems.
Other advantages of pragmatic trials are the ability to utilize existing data sources, and a higher probability of detecting significant changes than traditional trials. However, they may have some limitations that limit their credibility and generalizability. For example the participation rates in certain trials might be lower than expected due to the healthy-volunteer influence and incentives to pay or compete for

The authors of the Pragmatic Free Trial Meta identified RCTs published up to 2022 that self-described themselves as pragmatic. They assessed pragmatism using the PRECIS-2 tool that includes the domains eligibility criteria and recruitment criteria, as well as flexibility in intervention adherence, and follow-up. They found 14 trials scored highly pragmatic or pragmatic (i.e. scoring 5 or more) in at least one of these domains.
Studies with high pragmatism scores tend to have more lenient criteria for eligibility than traditional RCTs. They also contain populations from many different hospitals. The authors argue that these traits can make the pragmatic trials more relevant and applicable to daily practice, but they do not guarantee that a pragmatic trial is free from bias. Moreover, the pragmatism of trials is not a predetermined characteristic; a pragmatic trial that doesn't contain all the characteristics of a explanatory trial can yield valuable and reliable results.
