Pragmatic Free Trial Meta is a non-commercial open data platform and infrastructure that facilitates research on pragmatic trials. It collects and distributes cleaned trial data, ratings, and evaluations using PRECIS-2. This allows for a variety of meta-epidemiological analyses that examine the effect of treatment across trials of various levels of pragmatism.
Background
Pragmatic trials are increasingly recognized as providing real-world evidence for clinical decision-making. The term "pragmatic" however, is not used in a consistent manner and its definition and measurement require clarification. Pragmatic trials are designed to guide the practice of clinical medicine and policy decisions rather than prove a physiological or clinical hypothesis. A pragmatic trial should try to be as close as is possible to real-world clinical practices, including recruitment of participants, setting, design, delivery and implementation of interventions, determining and analysis results, as well as primary analysis. Other advantages of pragmatic trials are the possibility of using existing data sources, and a greater chance of detecting meaningful changes than traditional trials. However, these trials could have some limitations that limit their credibility and generalizability. For instance, participation rates in some trials might be lower than anticipated due to the healthy-volunteer influence and incentives to pay or compete for participants from other research studies (e.g., industry trials). The requirement to recruit participants quickly reduces the size of the sample and the impact of many pragmatic trials. Additionally certain pragmatic trials lack controls to ensure that the observed differences aren't due to biases in the conduct of trials.
