Pragmatic Free Trial Meta
Pragmatic Free Trial Meta is a free and non-commercial open data platform and infrastructure that facilitates research on pragmatic trials. It collects and distributes cleaned trial data, ratings and evaluations using PRECIS-2. This permits a variety of meta-epidemiological analyses that examine the effect of treatment across trials of various levels of pragmatism.
Background
Pragmatic studies are increasingly recognized as providing real-world evidence to support clinical decision-making. However, the usage of the term "pragmatic" is not consistent and its definition as well as assessment requires further clarification. Pragmatic trials are intended to guide clinical practices and policy choices, rather than confirm a physiological hypothesis or clinical hypothesis. A pragmatic trial should aim to be as close as is possible to real-world clinical practices, including recruitment of participants, setting, design, delivery and implementation of interventions, determination and analysis results, as well as primary analyses. Many RCTs that do not meet the requirements for pragmatism but have features that are contrary to pragmatism, have been published in journals of different types and incorrectly labeled as pragmatic. This can result in misleading claims of pragmaticity, and the usage of the term should be standardized. The development of a PRECIS-2 tool that provides an objective and standardized assessment of pragmatic features is a good start.
Methods
In a pragmatic study, the goal is to inform clinical or policy decisions by showing how an intervention could be integrated into routine treatment in real-world situations. Explanatory trials test hypotheses about the cause-effect relationship within idealised settings. In this way, pragmatic trials can have a lower internal validity than studies that explain and be more prone to biases in their design, analysis, and conduct. Despite their limitations, pragmatic research can be a valuable source of information for decision-making within the healthcare context.
The PRECIS-2 tool measures the degree of pragmatism in an RCT by assessing it across 9 domains ranging from 1 (very explanatory) to 5 (very pragmatic). In this study, the recruit-ment organization, flexibility in delivery, flexible adherence and

It is because adverse events are usually self-reported and are susceptible to delays, inaccuracies or coding differences. Therefore, it is crucial to improve the quality of outcome for these trials, in particular by using national registry databases instead of relying on participants to report adverse events in the trial's own database.
Results
While the definition of pragmatism may not require that all trials be 100% pragmatic, there are advantages to including pragmatic components in clinical trials. These include:

It is crucial to keep in mind that a pragmatic study should not mean that a trial is of poor quality. In fact, there is increasing numbers of clinical trials that use the word 'pragmatic,' either in their abstracts or titles (as defined by MEDLINE but which is neither sensitive nor precise). The use of these words in abstracts and titles could indicate a greater understanding of the importance of pragmatism,

Conclusions
As the value of real-world evidence becomes increasingly widespread the pragmatic trial has gained popularity in research. They are randomized trials that compare real world treatment options with experimental treatments in development. They include patient populations that are more similar to those who receive treatment in regular care. This approach can overcome the limitations of observational research, like the biases that are associated with the use of volunteers and the limited availability and codes that vary in national registers.
Other advantages of pragmatic trials are the ability to use existing data sources, and a greater probability of detecting significant changes than traditional trials. However, pragmatic tests may have some limitations that limit their validity and generalizability. For instance the rates of participation in some trials might be lower than expected due to the healthy-volunteer influence and incentives to pay or compete for participants from other research studies (e.g., industry trials). Practical trials are often limited by the need to enroll participants in a timely manner. Some pragmatic trials also lack controls to ensure that observed differences aren't due to biases in the trial.
The authors of the Pragmatic Free Trial Meta identified 48 RCTs self-labeled as pragmatic and that were published until 2022. They evaluated pragmatism using the PRECIS-2 tool, which consists of the eligibility criteria for domains as well as recruitment, flexibility in adherence to intervention and follow-up. They discovered that 14 of the trials scored highly or pragmatic practical (i.e. scoring 5 or higher) in any one or more of these domains and that the majority of them were single-center.
Trials with a high pragmatism rating tend to have broader eligibility criteria than traditional RCTs, which include very specific criteria that aren't likely to be used in clinical practice, and they comprise patients from a wide range of hospitals.
